If I have informed you that I have a waitlist please also fill out this form. I will track your progress based on the date it is submitted! Please read my couples, individual or workshop pages to make sure you understand how I work with prospective clients and to inform yourself of my rates and whether or not I take your insurance. If your insurance is not listed as one I take, then I can provide a Superbill for out-of-network services. You will need to contact your insurance company to see what rates they pay for mental health services or if you have out-of-network benefits.
